TAMI wiki registration broken, BorrowLog entry

Hi all,

The 'Request Account' page on the wiki is broken. I tried various input
combinations to check it wasn't a validation issue on one of the input
boxes, but it always returns a page that just says 'Database Error'.

I was signing up to the wiki because I wanted to add to the BorrowLog - I
borrowed a soldering iron that was hanging on the door by the electronics
bench, and I expect to return it by next Weds 24th.
